MEDANZ Membership Application and Membership Renewal Form

To Join MEDANZ or to renew your MEDANZ membership two simple steps are required:

1) complete the form below and

2) make payment by direct credit, cheque or Credit Card via PayPal (details below the form).

At  MEDANZ AGM on 14th April 2018 it was unanimously decided that: "Effective 1st July 2018 the annual Membership Fee increases from $35 for Full Membership to $50, Family Membership from $15 to $30"

Your membership will not be activated or renewed unless we have received both your details and your payment.

 

Membership categories are as follows:

Full Membership NZ$50.00 
Full subsidies at specified MEDANZ events.
Full voting rights
Eligible for election to Committee
May perform in MEDANZ shows
Eligible to apply for grants and scholarship funds
Have access to members area of MEDANZ website and MEDANZ Facebook group
Quarterly Newsletter sent by post and available online

Family Membership NZ$50.00 
+ $30 /extra family member - $ 24 before the END of July 2018.
Open to all people related by blood, legal or de facto marriage living at the same address.
At least one member of the family must be a full member of MEDANZ.
Each person shall have the rights of a full member but the household will receive only one newsletter.

MEDANZ is working hard on reducing its environmental impact and making information accessible to members more quickly by utilising online systems.

As a result the newsletter is now uploaded on the website and notifications are emailed to members to advise them to download it (including instructions on how to do this).

One hard copy of the newsletter will be sent to each household. You may choose not to receive hard copy newsletters by ticking the box below.
 No Postal Newsletter

The MEDANZ Membership List is issued annually at the AGM to members present (including apologies) and on written request by members to the committee. If you require your name to be excluded from the list, please contact the committee.

In addition MEDANZ publishes a version of the membership list on the members’ area of the website to promote networking between dancers. If you require your name to be excluded from the list, please contact the committee.

MEDANZ has been formed to provide a support network for our dancers, drummers and dance friends. Any information given on the form above will be treated as confidential and will not be distributed unless specifically agreed to by you.

Payment Details

Please remember that your membership will not become active unless payment has been received.

 

Our preferred payment method is by Direct Deposit to:

Ref: SUB +”YOUR NAME” + "Your membership number"

If you are a new member or do not recall your membership number please just include "SUB" and your name.
